Exclusive Services for Sullivan Place

Transportation

Transportation will be arranged to and from grocery stores and other markets around Saint Louis for the residents of Sullivan Place. This service is for residents of Sullivan Place only. Any resident interested in going on any of these scheduled trips will need to notify CREATE at least three days prior to the trip. It is the responsibility of the resident to be at the appropriate point of departure at the scheduled time and also to board the shuttle at the scheduled time. The driver will not be responsible for passengers.

CREATE will sponsor weekly trips to various places of interest around Saint Louis. These trips will occur every Friday. In most cases, these excursions are offered at no cost or a reduced cost to residents. Any resident interested in
participating in these scheduled excursions must notify CREATE prior to the activity by signing the particular sign-up sheet for the event. These excursions are for Sullivan Place residents only. Residents will be required to sign paperwork that indemnifies CREATE while the resident is on the trip. Weekly trips will be to a local bowling alley, a local mall, a local casino or a local cultural museum.

Currently, Meals - On - Wheels is the provider of meals for the residents at Sullivan Place only. Any resident interested in this service may register through CREATE and may be eligible to receive meals at no cost or a reduced cost depending on income. Due to changes in service areas for Meals-On-Wheels, deliveries have been sporadic. These issues are being resolved. Additionally, CREATE has partnered with a local church food pantry allowing residents to receive emergency food that will feed a family of two for approximately one week. A confidential form used to verify income levels will need to be filled out in order to receive this service.

Currently the Community Health-in-Partnership Services (CHIPS) representatives are on site at least once a week providing medical and social
services. Our goal is to have them on site twice a week. This service will be for Sullivan Place residents only. Services provided by CHIPS while on site are free to residents. Other services that are provided at the CHIPS
offices will be charged to the resident's insurance provider.  In addition, Chiropractors and Podiatrists will periodically provide free examinations and check ups. These services will be free to residents. We will attempt to have regularly updated schedules posted around the premises so residents know when each doctor will be visiting.

Occasionally, CREATE will sponsor/host a meeting or informative seminar conducted by a professional in a specific field. These seminars are free and are for Sullivan Place residents only.
